These should be deleted in hijack this:

O4 - HKLM\..\Run: [safqtzdA] C:\WINDOWS\safqtzdA.exe
Next run spybot in safe mode, you'll want to first disable system restore:
Right click on my computer and select properties
Next click on system restore and uncheck protect my system.
then boot into safe mode and run the spybot or other anti-spyware program.
Once done post another log so we can have a look
